As of The Boys Season 4, Episode 3, we haven’t gotten an explanation for the worm in Butcher’s head. So far, it has appeared when he is in duress or physical pain, but it remains to be seen if that’s a good or a bad thing.

The worm is almost definitely associated with Butcher’s usage of Temp V. It’s possible he was infected by a parasite, and the Temp V caused it to mutate. We could be seeing an awkward situation where Butcher has a sentient worm inside him and has to deal with even more complications.

The worm might be an attack from a yet unseen supe. We’ve seen supes with a wide array of powers, so it’s not outside of reason that one could have some sort of remote-control parasite power. If this is the case, a supe could track or attack Butcher without him even knowing.

Our last theory is that the worm may be a fail-safe mechanism implemented by the creators of Temp V. It could be designed to monitor or control the users of the drug, ensuring that they don’t become too powerful or turn against their creators. Given Vought’s tumultuous relationship with Homelander, we wouldn’t be surprised If this is the case.

Of course, in The Boy’s, there’s always a twist. With the show’s habit of turning what we think we know around on us, we’re likely shooting in the dark when it comes to what Butcher’s worm may be. Fortunately, we should know whether it’ll be his salvation or damnation in the coming weeks.
